ASSUMPTION OF RISK, WAIVER, AND RELEASE AGREEMENT
CAROLINA MOVEMENT
In consideration of Carolina Movement ("CM") permitting my attendance and participation in any activity, training, event, or gathering hosted or sponsored by Carolina Movement (the "Event") — specifically the event titled IMPACT RETREAT held in ATLANTIC BEACH from APRIL 15TH, 16TH, AND 17TH, 2026 — on behalf of myself, my heirs, and my personal representatives (collectively, "I," "me," "my," or "myself"), I acknowledge and agree to the following Assumption of Risk, Waiver, and Release Agreement (the "Agreement"):

REPRESENTATIONS AND ACKNOWLEDGEMENTS
I acknowledge and accept that:
(i) I have been made aware of the global pandemic of COVID-19, the proper precautions to take, and the risks associated with COVID-19;
(ii) I understand that social distancing is a recommended safety measure;
(iii) I have had the opportunity to remain at home or decline participation in the Event;
(iv) If I feel sick or unwell at any time during the Event, I will immediately remove myself and notify the appropriate CM contact;
(v) I have conducted reasonable diligence and am not currently demonstrating symptoms of COVID-19;
(vi) I am in satisfactory health and able to fully participate in the Event without harm to myself or others;
(vii) By participating in the Event, I may be at risk of exposure to illness, including COVID-19;
(viii) I agree to comply with any local or event-specific mandates regarding quarantining, masking, distancing, or vaccination.
I understand that Carolina Movement may assess risks and adjust Event participation guidelines accordingly. As a condition of attending the Event, I agree to comply with all safety requirements and restrictions, including but not limited to face coverings (unless medically exempt), distancing, and hygiene protocols.
I further understand that failure to comply with these expectations may result in my removal from the Event without refund or credit.

ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY
I voluntarily assume all risks of injury, illness, or death resulting from my participation in the Event. Accordingly, I release Carolina Movement and its directors, officers, employees, contractors, volunteers, and agents (collectively, "CM Parties") from any and all claims, demands, damages, liabilities, and expenses (including legal fees) arising from or related to my participation in the Event.
This release includes, without limitation, any claims related to exposure to COVID-19 or failure to follow safety protocols.
I further acknowledge that COVID-19 conditions may change, and CM may reasonably amend this Agreement or its policies at any time. I agree to review ALL CAPS EVENT WAIVER URL no later than seven (7) days before the Event to ensure I am aware of any amendments. CM may optionally notify me of updates by email.

INDEMNIFICATION
I agree to indemnify, defend, and hold harmless CM Parties from any and all claims arising from:
(i) my breach of this Agreement;
(ii) any transmission or alleged transmission of COVID-19 by me to others during or in connection with the Event.

PHOTOGRAPHY / VIDEO RELEASE
I grant Carolina Movement the right to use photos, videos, and recordings of me for promotional, educational, and informational purposes without compensation. I waive any right to inspect or approve the final use of such materials, which may be edited or altered at CM’s discretion. I understand these materials become the property of CM and may be used in future publications or media.

MISCELLANEOUS
Interpretation. This Agreement shall be interpreted as broadly as allowed by applicable law.
Severability. If any part of this Agreement is found unenforceable, the remainder will remain in full force.
Amendments. This Agreement may only be modified in writing and approved by CM.
Assignment. I may not transfer this Agreement without CM’s written consent. This Agreement binds and benefits CM and my heirs, legal representatives, and permitted assigns.
Electronic Signature. Electronic acceptance (including checking a box) will have the same legal effect as a handwritten signature.
Entire Agreement. This document contains the full agreement and supersedes all previous communications regarding its subject matter.

By checking the box titled “I agree to these terms,” I confirm that I have read and understood this Agreement, had the opportunity to consult with legal counsel if desired, and agree to its terms freely and voluntarily.
